At Trinity Medical, we understand that managing your healthcare involves more than just appointments. We offer a range of administrative services to support your medical needs, handled with the same care and efficiency you'd expect from all aspects of your care at Trinity Medical. All administrative requests are subject to a reasonable turnaround time. Please contact us in advance if you have an urgent requirement. All administrative services are available at our Carshalton Beeches clinic, serving patients across Sutton, Wallington, Banstead and South London.

Passport Applications and Countersignatures
Trinity Medical GP does not routinely provide passport countersignatures as a medical service.
HM Passport Office requires a countersignatory to have known the applicant personally for at least two years. A doctor cannot normally countersign a passport simply because the applicant is their patient; the doctor must know the applicant well personally and meet the current HM Passport Office eligibility requirements.
Being registered with or having attended Trinity Medical GP does not therefore, by itself, enable one of our doctors to countersign a passport application.
Where a Trinity doctor independently meets the required criteria, a passport countersignature may be provided at the doctor’s discretion and the standard £50 certification fee will apply.
Certification and Witnessing
Trinity Medical GP can only certify, verify or witness documents where it is appropriate for a medical practitioner to do so and where the doctor is satisfied that the relevant identity, document or information can be properly verified.
Please bring the original document and appropriate photographic identification where applicable.
We cannot certify or witness a document where the receiving organisation specifically requires a solicitor, notary public, commissioner for oaths or another designated professional.
Trinity Medical GP does not provide notarial services or legal advice.
Medical Forms, Letters and Reports
Administrative fees cover completion of the document, letter or report only. If a clinical assessment is required before the doctor can safely or appropriately complete the request, a GP consultation will also be required and charged separately.
We will not sign a document or provide a medical opinion unless we have sufficient information to support what we are being asked to confirm.
All forms, reports, certificates, document certification and signature witnessing are undertaken at the clinician’s discretion.
