Not every GP visit needs to start with illness. Our Proactive Home Health services are designed for people who find it difficult to attend the clinic and would benefit from taking a broader look at their health, medication and independence.
Sometimes the most valuable thing we can do is take the time to look at the whole picture before a problem becomes a crisis.
Whether you would simply like a general health review at home, or are concerned about an older relative who is becoming frailer or finding everyday life more difficult, we offer two levels of assessment.

Home Health Check Visit - 45 minutes | £450

Designed for patients who are unable to attend the clinic but would benefit from a more proactive review of their health.

This extended assessment includes:

Comprehensive GP consultation
Medical examination
Medication review
Blood pressure, pulse and oxygen saturation
Ear examination for wax
Blood draw where clinically indicated (laboratory fees charged separately)

Ideal for reviewing long-term health conditions, monitoring ongoing health and identifying concerns before they become more serious.

Comprehensive Home Health Assessment - 120 minutes | £995

Our Flagship Healthy Ageing Service

An in-depth GP-led assessment for older adults, housebound patients and those living with frailty or complex health needs.

In addition to everything included in the Home Health Check Visit, this comprehensive assessment includes:

Detailed medication and polypharmacy review
Falls and frailty assessment
Mobility and functional assessment
Activities of daily living and independence
Nutrition and continence review
Memory and mood screening where appropriate
Discussion with family or carers (with consent)
Future planning, including Lasting Power of Attorney and advance care planning where appropriate
Blood draw where clinically indicated (laboratory fees charged separately)
Dedicated follow-up consultation to review results, answer questions and agree a personalised care plan

This assessment is designed to identify concerns early, support independence, identify concerns early, support independence and help patients and families plan ahead with greater confidence.

Requesting a Proactive Home Health Visit

Home visits must be requested in advance and are not bookable directly online. All requests are reviewed individually to ensure they are appropriate, safe, and feasible.

You will be contacted to confirm whether a home visit can be offered
If approved, next steps will be provided.

Coverage Area

Our standard Proactive Home Health service covers a 3-mile radius from our clinic in Carshalton Beeches. For locations outside this range, additional travel charges may apply, and these will always be confirmed in advance. Contact us today to receive a quote.

For the first time.I can truly say that
I enjoyed a medical visit to a surgery.
The wonderful Dr Maria Carrasco
With so many years of experience in large numbers of people in her practice
.Really listened to me.
I had been feeling so worried as my condition
seemed to be getting worse A really horrendous experience.I felt the medical profession were letting me down completely .
In leaving me for nearly four months in constant pain And only sending in a referral to a specialist in the last four weeks .With long waiting lists .
So you can imagine how I felt when the amazing Dr Maria Carrasco phoned the Laboratory on my behalf regarding samples.
And outlined a treatment plan for me
it was like coming out of a nightmare .She listens to her patients .
And I felt for the first time for months that she understood what I was going through and that she was going to take the right action for me.

And would do everything in her power
To get me the correct treatment to put an end to my problem The relief was instant and I floated on air out of the surgery.
Then I started phoning everyone that I knew who were worried about health.problems to Come to the Trinity surgery and get help .
I had the most
wonderful consultation that I have ever had in my long life with a caring professional .who put my mind at rest immediately .
