New To Trinity Medical?
Whether you’re booking your first private GP appointment or looking for a new home for your healthcare, we’re delighted to welcome you to Trinity Medical.
There’s no lengthy registration process — simply book an appointment online or by phone and we’ll take care of the rest.
Before your first appointment, you’ll receive a registration form to complete, which captures some essential information including your medical history, NHS number and some other important information. We ask that this is returned to us prior to your appointment so that your care can begin without delay.
Depending on the type of appointment you book, you may also receive a short health questionnaire, which helps Dr Carrasco make the most of your time together.
Your appointment will be unhurried and focused entirely on you. Dr Carrasco will take the time to understand your concerns, your history, and your health goals — so that your care at Trinity Medical is truly personalised from day one.
Prescriptions & Referrals
Private prescriptions and referral letters are available as part of your GP consultation at Trinity Medical, where clinically appropriate — there is no additional charge for these within your appointment.
Please note that prescriptions and referrals are only available to patients who have had a consultation with Dr Carrasco. We are unable to issue prescriptions or referral letters without a prior clinical assessment, as this ensures your care is safe, appropriate, and tailored to your individual needs.
We do not currently offer repeat prescription services. If you require ongoing medication, we recommend discussing this with Dr Carrasco during your consultation, who can advise on the most appropriate pathway for your continued care.
Cancellation Policy
We require at least 48 hours notice to cancel or reschedule an appointment. Cancellations made within 48 hours will be charged at 100% of the appointment fee, appointments cancelled before this window are fully refunded.
